High-cost limit for healthcare

The high-cost limit is an economic safety net for those whose need of healthcare is great. The limit as to how much is to be paid in patient’s fees before receiving a free card for medical care (and a certain amount of dental care) is determined by The Swedish Parliament. It is your responsibility to keep track of your right to a free card. Currently, the limit is set at SEK 900 per 12-month period.

When your costs for medical care and treatment have reached SEK 900 within a 12-month period, you have the right to a free card for the rest of that 12-month period. For example, if you have paid SEK 840 kronor, you pay only SEK 60 for the next appointment – then you have the right to a free card. The free card is valid throughout the country.

Note that not all fees are covered by the free card.

In Halland, there is also a monthly high-cost limit set at SEK 400 per calendar month.

The high-cost limit includes
Patient’s fees for the following healthcare services are included in the high-cost limit when the healthcare is provided by the County Council or by private healthcare financed by public funds:

- medical care
- medical treatment
- house calls
- telephone consultations
- fees for calling prescriptions in to a pharmacy
- fees for a TNS-device (only Halland’s County Council)
- necessary dental care for certain elderly and disabled people, as well as others*
- dental care in connection with the treatment of an illness for a limited period of time*

When you receive a free card, the above healthcare services are free of charge as long as the free card is valid.

* the same fee as in outpatient healthcare applies to this dental care.

The high-cost limit does not include:
Certain fees may not be included in the sum that gives you the right to a free card. Examples of fees not included in the high-cost limit are:

- medical examination with a mammography
- gynecological medical examination
- vaccinations*
- providing certification*
- fees for ”missed appointments”*
- medical fees when admitted to a hospital

* Fees may vary between the County Council’s healthcare centers and the publicly funded private healthcare centers.

You keep track of your right to a free card
It is your responsibility to keep track of your right to a free card. In order to receive a free card you must be able to show that you have paid SEK 900 for medical care and treatment during a 12-month period. There is no monetary refund if you have paid more than SEK 900 when requesting a free card.

You can request a free card at such locations as healthcare centers and doctors offices. Each time you pay the patient’s fee, request that a notation be made in your high-cost card by the healthcare personnel. The fees should subsequently be added up. You can also use receipts as a basis for receiving a free card.

The card is valid from the date of issuance and 12 months forward from earliest recorded appointment at which you choose to use it. The free card is valid throughout Sweden. You must have your free card with you at every appointment, otherwise you must pay for the treatment. The free card does not apply for unpaid patient’s fees (invoices and in-payment cards) for appointments prior to the date the free card was issued.
